Accident Reporting - Supervisor's Guide

(12 min.) Accident investigation is an important part of a supervisor's responsibility and accident prevention. An investigation answers the questions of who, what, when, where, how, and why. A good investigation obtains as much information as possible about the accident; it may also include photographs, sketches, witness statements, and quite possibly include investigations conducted by engineering and maintenance personnel. Supervisors already know how to conduct investigations. This program will concentrate on the importance of conducting a thorough investigation and why the information is necessary. When you understand the purpose and reasons for the investigation and how the reports are used, it will make you a better supervisor and actually reduce the time you spend writing reports.

Topics included in this safety video are: the report form, the injured employee must never fill out the report, identify the employee correctly, employee's social security number, length of service, time of day, date the accident was reported and the actual date of injury, what date did the employee visit the doctor, was the treatment authorized, number of injuries or illnesses that the employee was involved in during the previous fiscal year, list all the witnesses to the accident, if there are no witnesses, description of the accident, identify the part of the body injured, the agency of accident space, if a piece of equipment contributed to the accident, unsafe act section, your opinion as to why or how the unsafe act was committed, corrective action taken or scheduled, approval signatures section.
